It is the policy of Hidden Treasures Thrift Store (The Store) that any children ages 14 and 15 must have
signed permission from their Parent or Legal Guardian to volunteer in The Store. In addition, any
children ages 13 and under must be accompanied by a parent or approved adult at all times while
volunteering in the store. The Store will routinely run background checks on all volunteers 14 and over.

By signing this form, I give permission for my minor child to volunteer at The Store. I understand that
there will may be employees from Masterworks of Minneapolis on-site at The Store during this time.
Masterworks exists to provide jobs to those with barriers to employment who may have a criminal
background. It is the goal of The Store to help in providing employment to people with these difficulties
and the policy of The Store that any Masterworks employees working for The Store will not have offenses
in their background that are sexual or violent in nature.
